This position supervises and monitors the delivery, management and coordination of care and services provided to patients via telecommunications technology within the domain of ambulatory care nursing.
Performs telehealth clinical assessment, health education and utilization management to ensure prompt identification and most appropriate clinical disposition response for patients reporting symptom-based concerns.
This position supervises and monitors the process and outcomes of the Patient Access Center to ensure patients who are reporting symptoms are accessing the most appropriate setting of care safely, efficiently and timely.
Utilizes Scope and Standards of Practice for Professional Telehealth Nursing while exercising critical thinking, nursing judgement and standard triage guidelines to ensure health needs and concerns of a patient, group, or population are met.
Functions independently to evaluate complex patient scheduling needs to ensure patient is transitioned to appropriate level of care and that the organization optimizes available same day appointments.
The Triage Nurse will also provide excellent customer service while collaborating with patients, family members, caregivers, and other health care professionals in the delivery of Telehealth Nursing Practice.
Communicates effectively using a variety of formats, tools, and technologies to build professional relationships and to deliver care across the continuum.
Educates non-clinical Patient Access Center employees to ensure clinical decisions are elevated to the appropriate clinical resource.
The position reports to the Director Clinical Nursing Practice with a matrix reporting relationship to the Manager Patient Access Center.
Graduate of an accredited school of nursing (Required)
PA Registered Nurse License (Required)
