Mobile community-based position for members with complex physical health, behavioral health and psychosocial needs. Reliable transportation is required and bilingual Spanish-speaking skills are strongly preferred. Responsible to assist members who require care coordination and conducts assessments that include behavioral, clinical, social, and environmental concerns or needs.
Adult members will be followed in their community, place of residence, and in facilities. This position coordinates programs, services, and facilitate communication between the member’s physicians, physical and behavioral health clinicians, and community-based services. Provides behavioral health support, education and supervisory services to colleagues, Reading Health System departments, and to other providers and agencies as needed.
