The Palliative Care Service (PCS) seeks to provide an integrated approach to care for patients and families who are facing serious illness, by focusing on quality of life and relief of suffering.
The Palliative Care Coordinator works as part of the PCS Team, providing both direct clinical care as well as support for operational goals.
-Coordinating the ambulatory panel of patients including pre-visit planning/ assessment upon arrival
-Inpatient section coverage
-Patient and family support
-Tracking metrics for ambulatory
-The main responsibility is to increase the team’s capacity to manage referrals and to follow up on patient care needs.
-As the PCS seeks to integrate evidence-based clinical practice within the hospital continuum of care, the nurse coordinator participates in educational processes and maintains a close interdisciplinary presence.
-In collaboration with the PCS Team, the nurse coordinator supports growth and development of the program, through activities in: research, quality improvement, operational and clinical metrics, community education, and hospital-wide initiatives.
-Current RN License.
-Master's Degree preferred.
-Hospice and Palliative Care certification, ICU experience preferred.
-Knowledge about home care, hospice, palliative care, advance directives, pain management and symptom control.
-Ability to have end of life conversations while being supportive of patient and family.
-Palliative, hospice, oncology, heart failure and/or nurse navigator experience preferred
