This position will be responsible for a Friday evening and corresponding weekend on a rotation basis, a rotating holiday every year, and picking up open shifts.
The Pharmacist is responsible for assisting in the planning, organization and direction of clinical pharamceutical services to achieve high- quality standard-of-care in pharmacy services to the critical care areas. Provides specialized pharmaceutical services to imporve drug usage and therapeutic outcomes for critical care patients. Directly covert from IV to oral therapy per hospital protocol. Develop formulary assessment material and play a key role in evaluation critical care related formulary items. Continually improves the delivery of pharamceutical care to patients in the critical care areas and serves as a resource for other Pharamcists in the department who service the critical care areas. Participates on multidisciplinary teams related to critical care. Participates in the development and delivery of critical care related continuing education programs for the Pharmacy department. Participates in the preception and education of pharmacy students. Achieves and maintains satisfactory working relationships with physicians, nurses, dieticians, respiratory therapists, pharmacy staff, other clinicians and hospital staff.
Must have a Pharmacy degree (Bachelor of Science or Pharm.D.) Successful completion of a post-graduate residency or equivalent experience in critical care is required. Possess a valid Pennsylvania State License from the Pennsylvania Board of Pharmacy or be board eligible for Pharmacy Licensure.
