The RN Case Manager is responsible to assist in the development, planning, coordination and administration of the activities of discharge planning for patients in behavioral health unit.�
Education: - Bachelor's degree in social work, psychology, human services, or nursing. Equivalent experience of four years in acute medical-surgical, psychiatric, or home health nursing. Requirements: - Two years experience providing discharge planning or other relevant service in an acute medial or psychiatric hospital. - Demonstrated knowledge of communicty resources, discharge planning, patient linkage process, Medicare and other regulatory agency rules/regulations. - Knowledge of medical terminology, charting skills, and familiarity with a medical setting. - Excellent communication and interpersonal skills. - Proficiency with Microsoft Office applications (Word, Excel, and Powerpoint).
