Supports physicians in office during clinic and providing education to patients during time of visit.
Coordinates care for Pulmonary Hypertension patients ensuring appropriate labs, testing, and procedures are scheduled and completed.
Facilitates triage for Pulmonary Hypertension patients to ensure patients receive timely follow up and necessary education.
Responsible for population management and maintaining patient metric data for Pulmonary Hypertension patients/clinic.
Facilitates weekly Pulmonary Hypertension clinic conference to review patient cases.
Processes prior auths for patient meds/equipment.
RN. Bachelor's Degree in Nursing (BSN) preferred. EPIC experience preferred. 1 year experience working as an RN in ambulatory setting desired. Excellent communication, leadership and organizational skills.
