  • Overview

    The HIM & Specialist plays an integral part in the retrieval, preparation, scanning and maintenance of medical records to ensure timely timely availability for continuity of patient care, coding and reimbursement activities, physician chart completion, and permanent record storage and retrieval.  This position ensures that medical records are maintained in accordance with Medical Staff Rules and Regulations, state and Federal guidelines, and JC accrediting standards.  All work will be carried out in accordance with the Chestnut Hill Hospital's Health Information Management department and Tower Health approved policies and procedures.

    Including, but not limited to the following Job Competencies

    • Accurately Perform Chart Reconcilliation, Retrieval and Processing to include, Prepping, Scanning, Indexing, Validating and QAing all Medical Record Documentation
    • Accurately analyze all applicable medical record documentation for physician completion
    • Handles high volume customer phone calls and complies with all internal/external requests for release of Protected Patient Health Information from patients, physicians, hospitals and other outside entities, while adhering to hospital & departmental policies, state and federal regulations related to HIPAA and appropriate Release of Information.
    • Death Certificate Processing
    • Duplicate Medical Record Number Processing
    • All other HIM Related Functions as assigned.





    High school Diploma/ GED required.


    Must possess relevant experience in clerical duties sufficient for individual development to the level of independent functioning with minimal direction.  Must posess at least 1 year of experience in an acute care hospital setting, preferably in the area of Medical Record Processing and/or Release of Information.
